New patients

We're glad you're coming to see us. Here's how to get ready, so your first visit goes smoothly.

What to bring to your appointment

  • 1Your medical recordsBring your records with you, or ask your referring doctor to fax them to our office before your appointment.
  • 2X-rays from the emergency roomIf you were seen in an emergency room and had an X-ray, please bring the films and the report.
  • 3All of your medicationsBring the medications you take, so we can make sure your records are complete and accurate.
  • 4Insurance card and photo IDWe'll need both to set up your account.
  • 5Your completed formsFilling out our forms at home saves time at check-in. You're welcome to complete them at the office, too.

Having everything with you helps us record all the important information and plan the best care for you.

Insurance & payment

Insurance plans change often. Please call us at (864) 877-0779 before your visit, and we'll confirm whether we participate with your plan.

Please bring your insurance card to every visit.

Our payment policy

  • If we participate with your insurance company, you're responsible for any copays and deductibles your plan requires. We can't waive them.
  • A referral or authorization doesn't guarantee that your insurance will pay. The cardholder is responsible for payment.
  • If you don't have insurance, payment is due at the time of service unless you've made other arrangements with our office before your appointment.
